Required starting documents

To ensure we provide the very best care for your child, we require a small number of documents prior to their start date. These help us meet safeguarding requirements and understand your child’s individual needs from day one.

  • Completed registration form
  • Child’s birth certificate (copy)
  • Proof of home address
  • Emergency contact details
  • Medical information and immunisation record
  • Signed consent forms (trips, photos, medication etc.)
